Sample Collection Instruction

Step 1 – Choose Your Tap

Select the tap you use most often for drinking water
(e.g., kitchen sink or refrigerator water dispenser).
Use this same tap for the entire collection.

Step 2 – Get Ready

DO

• Wash hands before sampling

• Put on the included nitrile gloves before collecting your sample
  (soaps, lotions, and other personal products may contain PFAS — the gloves help prevent contamination)

• Hold the vial only by the sides

DON’T

• Touch the inside of the vial or cap

• Use lotions, cosmetics, sunscreen, or insect repellent before sampling

Step 3 – Collect Your Water Sample

1. Turn on cold water and run for 2 minutes.

2. Reduce to a gentle flow.

3. Fill the vial to the line.

4. Close the cap tightly.

Step 4 – Complete Chain-of-Custody

• Confirm the Kit ID matches the vial label
  (the Kit ID is printed on the outside of the return mailer and on your Chain-of-Custody card)

• Record the collection date & time

• Sign the card

Please include the completed Chain-of-Custody card in the return mailer with your sample so we can process your results without delay.

Step 5 – Package & Mail

• Place the vial into the zip bag and seal the bag completely
  (this helps prevent contamination and leaks)

• Insert the sealed bag and completed Chain-of-Custody card into the return mailer

• Mail within 24 hours
